Web-Based Family History Questionnaire

Since 1996, we’ve been providing research institutions and clinical genetic services worldwide the ability to track family history data. Recently, we’ve developed a method to custom design web data entry forms that capture family history data and dump the data into the Progeny database. This is ideal for getting patients to complete the family history questionnaire prior to making their on-site visit. It can also be used to allow collaborators to remotely enter data into the Progeny database via this web form.

Record Unlimited Number of Generations

You can design the forms to capture as many generations of family members as you want. You can also design the form to capture as much detailed information as you want.

Produce a Pedigree Image for the Patient

After the patient has completed the questionnaire, you can generate a picture of the pedigree for them to print out or save as a jpg on their PC. This isn’t a requirement, but is left up to you to decide if you want to offer this feature or not.

Download Data from Questionnaire into the Progeny Database

Once the questionnaire is completed, you can retrieve the data and dump it directly into the Progeny database. The pedigree can then be reviewed and updated once the information is verified with the patient.

Custom Programming

We also provide custom programming options if you want specific pages to pop-up for the user based on information they enter, such as whether or not they are at high, medium, or low risk for breast cancer. This criteria would be based on algorithms you provide.
